2 questions about games and firmware

1: Is there a list of which games require which firmware anywhere?

2: Do game disc come with firmware updates in them, if so, can you opt out of installing them(And obviously not being able to play).
In the case of yes, they do come with updates, will they only update the system to whatever the specified firmware is and no further?
